TRAVEL DOCUMENTS
: A United States citizen must be in possession of a valid passport. You may apply for a passport at the nearest Federal District Court, Post Office, or other authorized agencies. U.S. citizens must obtain a Visa from the Embassy of the United Republic of Tanzania, 2139 R Street N.W., Washington, D.C, or Representative of The United Republic of Tanzania to the United Nations, 201 East 42nd Street, Suite 1700, New York, NY 10017, (212)-972-9160. The cost for a single entry is $50. You must have at least six months of validity left on your passport at the time of the Summit. Many countries and airlines do not permit travelers with less validity. Please visit the Tanzanian Embassy website for Visa forms and other travel information at .http://www.tanzaniaembassy-us.org/visa/visa.html Non-U.S. citizens must check requirements with Embassy of the United Republic of Tanzania.

HEALTH REQUIREMENTS: At the time of booking, you will be advised of any inoculations that are necessary. Required inoculations must be recorded on a valid vaccination certificate and must be in your possession. We also recommend that you consult your doctor or the Center for Disease Control in Atlanta, Georgia at (404) 639-3311 http://www.cdc.gov/travel/eafrica.htm in regards to your itinerary and required inoculations. Travel in Africa may include adverse, strenuous or other conditions such as temperature fluctuations. Please bring these conditions to the attention of your physician prior to travel.

RESERVATIONS (Charter): Early registration is strongly recommended. All payments must be made in US Dollars. To make your reservation, just click on http://www.thesullivansummit.org; an initial deposit of 10% is required for those who choose to pay by installment. Payment can be made by Credit Card (Visa, MasterCard, Discover or American Express) Check, Bank Draft, Cash or Wire Transfer. You also have the option to download a blank Summit VIII Registration form (in PDF format) complete, sign the form and include the full payment or the initial down payment of 10% of the total cost. You will only receive your airline tickets and hotel booking upon full payment. Roommate choice will be confirmed upon full payment of both parties. You do not have a reservation until we have received your full payment. If the Summit sells out before you have completed your payment, you will be deemed to have cancelled and cancellation policies will apply.




Payment Methods

PAYMENT IN FULL

Make payment in full by credit cards (Visa, MasterCard, American Express and Discover), Check, Bank Draft, Wire Transfer, or Money Order (make payable to Leon H. Sullivan Summit) in US Dollars only. NO PERSONAL CHECKS WILL BE ACCEPETD AFTER MAY 15, 2008.

PAYMENT BY INSTALLMENT ONLY

Minimum payment on the first installment is 10% on the total price. An installment plan will be mailed to you disclosing the dates on Which the succeeding payments are due.

Upon receipt of full payment, charter delegates will receive airline and room confirmation.

For those wishing to pay by installment please be aware that your credit card will be automatically charged according to the dates listed above. Those wishing to cancel payment at anytime must provide written notification of cancellation and will be subject to cancellation policy.

Cancellation Policy

CANCELLATION POLICY (Charter): Leon H. Sullivan Foundation has to make various deposits in order to reserve accommodations for hotel rooms, transportation arrangements, airfare etc., for Leon H. Sullivan Summit VIII. If you must withdraw your reservation after you have registered and made payment, the following cancellation fees will take effect: Please note the Leon H. Sullivan Foundation will refund the following amounts:

January 1, 2008 – February 29, 2008 85% of payment received
March 1, 2008 – April 4, 2008 50% of payment received
April 5, 2008 – May 15, 2008 0 refund (no refund issued)

NO refunds cancellations after May 15, 2008 will be processed. No refund will be issued for any unused accommodations, missed meals or Summit events, or unused services of chartered transportation. Leon H. Sullivan Foundation has the right to cancel any individual registration payable to Leon H. Sullivan Summit prior to departure by refunding the full amount to the delegate. Leon H. Sullivan Foundation is not responsible for other expenses incurred by the delegate such as Visas, medical expenses, excess baggage non-official meals, room service, minibars, tips or any other trip preparation.

CANCELLATION POLICY (Non-Charter): The cancellation policy for non-charter delegates mirrors the cancellation policy, as detailed above for charter delegates.



Travel, Insurance & Responsibility

TRAVEL INFORMATION: A payment acknowledgement will be sent immediately upon receipt of payment along with an invoice for any balance remaining. Upon full payment (charter), your airline ticket, hotel booking, and travel itinerary will be sent along with general information such as reading and clothing suggestions, customs information, and weather.

INSURANCE: You may inquire with Northbank Travel at 602-468-1699 or toll free 800-775-0089 or send an email to [email protected] regarding a Travel Insurance Plan to protect you and your investment in the event of a sudden emergency before or during your trip. This plan offers coverage for trip cancellation or interruption, illness, injury and damage, loss or theft of luggage, as well as emergency assistance. Information on this insurance plan will be provided upon receipt of payment.

DISCLOSURE NOTICE: Your participation in any international travel has inherent risks and certain dangers may arise, including but not limited to the hazards of traveling in undeveloped areas, travel by various conveyances, forces of nature, political unrest and accident or illness in remote regions without adequate means of evacuation or medical facilities. Please be aware and understand that Leon H. Sullivan Foundation will not accept any liability for provision of medical services nor accuracy of medical care rendered. The Leon H. Sullivan Foundation will make its best effort to ensure that adequate measures are taken to avoid such circumstances.

YOUR PARTICIPATION IN THESE ACTIVITIES IS PURELY VOLUNTARY AND WITH FULL KNOWLEDGE THAT CERTAIN DANGERS ARE INHERENTLY INVOLVED AND YOUR PARTICIPATION IS AT YOUR OWN RISK. PLEASE NOTE THAT TRAVELERS WHO ELECT TO BRING SMALL CHILDREN AND ELDERLY WITH THEM TO LEON H. SULLIVAN SUMMIT ARE UNDER A HEIGHTENED DUTY TO PROVIDE NECESSARY HEALTH AND RELATED PRECAUTIONS FOR SUCH MINOR (S) AND ELDERLY. LEON H.SULLIVAN SUMMIT WILL ACCEPT NO RESPONSIBILITY FOR THE HEALTH OR WELFARE OF MINORS AND ELDERLIES.

SUMMIT REGISTRATION (Charter): Includes Air Travel Round trip, coach class from either Hartsfield-Jackson International (ATL), John F. Kennedy (JFK) or Baltimore Washington International Airport (BWI) to Tanzania.

ACCOMMODATIONS (Charter): Deluxe and first class or best available hotels will be used. All taxes and service charges are included. Rates are based on two persons sharing a twin bedded room. Members choosing single rooms will pay the single room upgrade fee. Single rooms are not always available; therefore the single room supplement quoted excludes single rooms in these locations. The right is reserved by Leon H. Sullivan Summit VIII to substitute hotels, when necessary.

MEALS: All official Summit meals are included in the Standard packages for the charter delegates and the non-charter delegates who paid for the entire five-day sessions. Refer to Summit schedule for details.

TRANSFERS (Charter): Transportation between the airport in Tanzania and your hotel is included in your charter package price. Transportation between your hotel in Tanzania and the summit conference center is included for all. Transfers are not included when arriving or departing by means other than the charter.

BAGGAGE (Charter): Arrangements include transportation and transfer of two pieces of baggage with a total weight not exceeding 100 pounds for checked baggage (each piece of checked baggage must weigh at least 50 pounds) and one piece of carry-on baggage must not exceed 30 pounds. Additional baggage is subject to space availability and will require an excess baggage fee of $60 per piece. Please note: On airline travel in Africa, baggage limits are strictly enforced. Different weight limits apply for travel not originating and terminating in the United States. Non-charter delegates should check with their airline for baggage limits.

Baggage and personal effects are at owner's risk throughout the conference, and baggage insurance is strongly recommended. Carrier liability for delegates' baggage is clearly stated on the passenger contract included on all tickets or can be found on file for inspection at the offices of the carrier. Liability for checked baggage in no event exceeds the actual loss incurred by the passenger, subject to proof of the amount of loss.

ARTIFACTS FROM TANZANIA (Charter): The same baggage rule applies to delegates bringing artifacts from Tanzania. A total weight not exceeding 100 pounds for checked baggage (each piece of checked baggage must weigh at least 50 pounds) and one piece of carry-on baggage must not exceed 30 pounds. As you shop for figurines and other cultural items please be mindful of the weight. No animals, fruits, plants, Cuban cigars, Ivory, and large paintings are allowed from Tanzania to the United States. Decorative African Canes may not be allowed as carry-on baggage. Please seek U.S., customs advise as to which items and quantities are allowed in the United States.

CHARTER PRICE EXCLUDES: Travel to New York JFK, ATL and BWI airport, passport, health certificate, Visa, airport and port taxes. All items of a personal nature including but not limited to laundry, wine, liquor (exception special occasions), non-official meals, room service, mini-bar items, tips for room service and items not on the regular menus, transport to restaurants and back, excess baggage charges, cancellation, health and accident insurances, private non-group transfers, all other incidentals and any items not specifically listed as “included” are not included in the tour price.

RESPONSIBILITY (Charter): Leon H. Sullivan Foundation and all cooperating agencies act only in the capacity of agents for the hotels, airlines, bus companies, railroads, ship lines or owners or other contractors providing the accommodations, transportation or other services. All coupons, exchange orders, receipts, contracts and tickets issued by Leon H. Sullivan Foundation are issued subject to any and all tariffs, terms and conditions under which any accommodations, transportation or any other services whatsoever are provided by such hotels, airlines, bus companies, railroads, ship lines or owners or contractors or by their person in employ. By the acceptance of tour membership and/or such coupons, exchange orders, receipts, contracts and tickets, the tour member agrees to the foregoing and also agrees that neither Leon H. Sullivan Foundation, nor any of its affiliates or subsidiaries, shall be or become liable or responsible for any loss, injury or damage to person, property or otherwise in connection with any accommodations, transportation or other services resulting directly or indirectly from any acts of God, dangers incident to the sea, fire, breakdown in machinery or equipment, or wars, whether declared or not, hostilities, civil disturbances, strikes, riots, thefts, pilferage, epidemics, quarantines, medical or customs regulation, defaults, delays or cancellations of or change in itinerary of schedules, or from any causes beyond the control of Leon H. Sullivan Foundation, or for any loss or damage resulting from insufficient or improperly issued passports, Visas or documents, and that neither the Foundation nor any of its affiliates or subsidiaries shall be or become the liable or responsible for any additional expenses or liability sustained or incurred by the tour member as a result of any of the foregoing causes. Leon H. Sullivan Foundation reserves the right to decline to accept or retain any person as a tour passenger should such person's health, mental condition, physical infirmity or general deportment impede the operation of the tour or the rights, welfare or enjoyment of other tour passengers. The passenger’s name in his or her passport must match the name on the airline ticket. A refund of unused land tour services is the limit of the liability of Leon H. Sullivan Foundation. The right is reserved to substitute hotels, and alter the itinerary or revise the places to be visited. The right is reserved to cancel the tour prior to departure, if any tour must be withdrawn prior to the passenger's joining the tour, Leon H. Sullivan Foundation is not responsible nor liable for defaults of those not directly under the control of the Summit organization or its affiliate organizations. Tour members are held responsible for being in sufficiently good health to undertake the travel to Tanzania and all subsequent or additional tours and trips therein.
